Real Life Choices is a self -directed approach that represents a system change. It provides services and supports based on an individual budget. The consumer and family can purchase services and supports to achieve the outcomes they have identified in their Plan of Care.
Real Life Choices is now in the process of bringing self-directed services to New Jersey residents with developmental disabilities.
The New Jersey Division of Developmental Disabilities is using the Family Support Center clearinghouse as a tool for families in Real Life Choices. Families will be able to not only find resources but also to see the agencies that have been approved by the Division of Developmental Disabilities to provide and receive payment for services.
