Self-Directed Survey Preparation
FSCNJ invites you to a Self-Directed Survey Preparation workshop: Presenters include representatives from New Jersey Developmental Disabilities Planning Institute & Advocates for Alternatives. As DDD moves to a completely Self-Directed system - These workshops are being provided to gain a better understanding of the needs assessment survey. This is an opportunity to learn how the Developmental Disabilities Resource Tool (DDRT) is used to help identify and determine an individual's level of need.
This presentation is for individuals eligible for Division of Developmental Disabilities (DDD) services. It is a forum to clarify the DDRT.This survey is now being conducted either online or by phone. A review of the DDRT will be presented, examples will be given of individuals who are currently self-directing, and time will be allotted for questions and answers.
